Hospice of Lenawee’s Social Work Team supports the Hospice patient and their family in a variety of ways. The Hospice of Lenawee Social Worker represents the patient’s wishes as a member of the interdisciplinary team. The patient/family unit, along with the social worker, assesses and decides on a course of care. The Social Work Team’s primary goal is to assist patients to continue living their lives as actively and meaningfully as possible.
