Hospice care can be requested by anyone, including:
- Physicians
- Patients
- Family Members
- Hospital Personnel
- Nursing Agencies

Admission Criteria:
- Individuals with a life expectancy of months rather than years, confirmed by the personal physician
- Individual resides in or near Lenawee County
- Individual and family choose hospice care

Payment of Services:
- Medicare, Medicaid, along with Blue Cross/Blue Shield and other private insurance programs cover most of the cost of hospice care
- If you do not qualify for Medicare or Medicaid, hospice can still be available to you at little or no cost – please contact us for information
- Gifts to Hospice of Lenawee allow us to offer services to all regardless of ability to pay
